Please read this policy before submitting any personal information. “Personal Information” is information that identifies you as an individual or relates to an identifiable person, such as name, postal address, telephone number, email address, credit card number, and social media account ID.

This policy outlines the specific purposes we collect personal information for, how we keep it secure, up to date and for how long. This covers data collected via email, telephone in person and through our social media pages.

We collect the following necessary personal data from an individual:

  • Medical History

  • Contact Information

  • Emergency Contact Information

 

The above information is collected from individuals in the form of a ‘Client screening questionnaire’. Medical history is collected in order for us to ascertain if an individual is suitable to attend one of our classes and if they require any specific modifications. An individual’s contact information; phone number and email address, is collected in order to have means to contact the individual regarding upcoming classes. Only when and if the client agrees will they be added to our mailing list in order for us to inform them of special offers and events. We will never share this information with any third parties unless there is medical reason to do so and consent has been given. An emergency contact is requested so that we have a point of contact to call in the event of an emergency.

How we keep data secure

We keep our Client screening questionnaires in a file in a locked room. The Client screening questionnaires of newer attendees, which are completed and returned via email, are kept on a password protected laptop. Client screening questionnaires will not be shared with anyone. No financial data is stored. However, these measures do not guarantee that your information will not be accessed, disclosed, altered or destroyed by breach of such precautions. By using our Service, you acknowledge that you understand and agree to assume these risks.

Client screening questionnaires of all individuals are kept for the duration that they continue to attend classes and for a period of 6 months after they stop attending classes when they will be destroyed. If an individual wishes to begin attending classes again after 6 months they must complete a new form to ensure information is up to date.

You can ask to be removed from our mailing list at any time by contacting us by email and making a request.
